Jacob "Jake" William Cook, 22, passed away September, 11, 2018 at home in Grand Junction, Colorado. Jacob was born to Christopher and Sharon Cook on December, 15, 1995 in Albuquerque, New Mexico. He is survived by his wife, Irene Slovian, step son, Lennon Slovian. Both parents, Christopher and Sharon Cook-Schultheis; brothers: Anthony "Tony" Cook and Jared Cook. Sister, Shannen Cook all of Grand Junction, Colorado. Jacob was a package handler for a local freight company. Services to be determined at a later date. Cremation has taken place.
